Vaccinations: All dogs must have current vaccinations against Rabies, Bordetella and DHLPP. Please note:getting a Bordetella vaccination is not a guarantee your dog won’t get Bordetella. Proof of vaccinations must be on file prior to admittance into A-Team K9 programs. A-Team K9 is not responsible for the upkeep of shots, or reminders for vaccination. Reminders for proof of current records will be requested as shot records near expiration. In the case records cannot be obtained after the third request, the dog in question can no longer participate in programs without proof of updated vaccinations. All dogs must be free of communicable disease for 90 days prior to admittance.Option 2
Flea & Tick Protection: All dogs are required to have flea protection. Each dog must be on a veterinarian-approved form of flea & tick control. Acceptable flea treatments are (but not limited to) Frontline, Frontline Plus, Advantage, K-9 Advantage, and Revolution. Any dog that shows signs of fleas, ticks, or other parasites will not be allowed on site, nor rendered service until a veterinarian's letter validates the condition has been cured.
Collars, Leashes, and Other: All dogs must be on leash when entering and exiting facilities. For off-site field trips, A-Team K9 reserves the right to gear all participating dogs with A-Team owned collars, leashes, bandannas, ecollars, vests, etc. that may be different from the owner's until the end of the field trip. A-Team reserves the right to secure and separate dogs as needed to maintain safety for all dogs, staff, and public.
Training Services: Acknowledgement each dog has its own, individual personality, history, and needs. A-Team K9 will deliver training sessions in the frequency and duration as agreed to in service package, but is not responsible for owner's portion of behavior reonforcement, effectiveness if length or frequency of training is not sufficient, or guarantees performance if other methods of behavior modification is used in conjunction with A-Team K9's training program.Owners agree to be involved, participate as required per individual training plan, and understand he/she and members of the household must follow training instructions without modification,
Service Fees: All fees for services may be paid in advance, and are due when services are rendered. Monthly reoccuring packages are debited the first of each month, and unused days never expire as long as a continued service plan is in place. Training fees are due at the time of booking, unless payment plans with direct debit authorization is in place. Failure to clear payment as arranged serves as automatic hold on services, including cancellation of services, until payment plus applicable fees are paid. Acceptable forms of payment are Paypal, Venmo, credit cards & cash. Cash and Venmo payments in full at time of booking are eligible for 3% discount.
I understand that by admitting my dog into A-Team K9's programs or services, officers, employees, and affiliations of A-Team K9 have relied on my representation of the temperament and health of my animal. For non-training services, I declare that my dog has not harmed any person or dog and has never shown aggression or threatening behavior of any kind toward any person or any other dog.

All dogs are required to dog receive the Bordetella (Kennel Cough) vaccine every six (6) months, and be up to date on Rabies, DHLPP, heartworm, and flea/tick shots and medication. I certify my dog(s) is in good health and is free from any condition(s) that could potentially jeopardize other dogs. Dogs that show signs of illness or injury under A-Team's care will be isolated and immediately dropped off, or required to be picked up ASAP. Dogs with stitches are not allowed back into non-training services until they are removed, or 10 days have passed for stitches that dissolve. Dogs that are seriously ill or injured will be transported to your designated animal hospital whenever possible, provided reasonable distance and condition of dog do not require emergency veterinarian assistance. Services will not be provided if listed requirements are not met.
I understand by having my dog(s) participate in service programs by A-Team K9, LLC, (s)he will be in close contact with people and other dogs of all varieties. Should my dog cause damage or harm, to any person, animal or property in any way; or should my dog in any way become harmed by any person, animal or property while in the care on or off the properties of A-Team K9, I agree to indemnify and hold harmless A-Team K9, LLC, it’s officers, employees and affiliations from any liability, costs, expenses or claims per this waiver of indemnity.
I understand if in my absence, my pet(s) should become ill or injured, or in need of veterinary care, A-Team K9 has my permission to consult with my veterinarian and any medical issues that require immediate veterinary attention. If I or my alternate contact are unavailable, my dog will be treated at the sole discretion of the A-Team K9 staff, employees, or associates, and I understand that I am responsible for all veterinary costs, including the transportation of my pet to and from the veterinarian.
I authorize the card on file to be used for emergency veterinarian expenses, and payment of products, subscriptions, and services.
